I assume that you are referring to the major routes. If they are being sent to the mainland for care and are low-risk, they usually will go via Ambulance aboard the ferry. Generally, critical patients that require specialty care will be transported by helicopter.
I am pretty sure that the ambulance has priority for travel and would be treated as having an ALT.

Emergency medical traffic does get priority on BCF. I don't know if transfers of stable patients would class as emergency, though.
In addition to BCF and air ambulances, the Coast Guard's hovercraft are occasionally used for island medivacs. I believe Penac did a medevac from Bowen last week. They're fast enough (~45 kts) to make it to the eastern gulf islands (e.g. Galiano, Mayne) without much trouble.
